  • Training (3 weeks): 9:00 AM – 5:30 PM ET
  • Post-Training: 8:30 AM – 5:00 PM ET, Mon–Fri
  • Time Zone: Must be able to work Eastern Time Zone hours (AZ candidates must adjust for daylight saving time)

About the Role:

BCforward is seeking a Care Management Support Coordinator II to join our client’s remote team. This role supports administrative care management functions in a fast-paced, member-focused environment. It's an ideal opportunity for professionals with healthcare and call center experience who are passionate about making a difference in members' lives.

πŸ” Key Responsibilities:

  • Conduct outbound member outreach and respond to inbound support needs.
  • Assist members with service coordination, care plans, and resource connections.
  • Educate members and providers on care plan procedures.
  • Send onboarding materials and welcome letters.
  • Maintain accurate, non-clinical member documentation.
  • Identify and refer members to SDOH-related programs.
  • Use multiple systems/applications to support member activities.
  • Collaborate in a team environment via chat and virtual tools.
  • Ensure compliance with state/regulatory documentation standards.

βœ… Required Qualifications:

  • High school diploma or GED.
  • 1–2 years of experience in a healthcare-related role (e.g., medical office, MA, health plan support).
  • 6+ months of experience in a high-volume call center.
  • Strong communication and multitasking skills.
  • Comfortable navigating multiple systems and resolving basic tech issues.
  • Excellent time management and self-discipline in remote settings.
